When patients contact their GP requesting a same day home visit for an acute need, the GPs can choose to refer the patient onto the acute home visiting service. Our highly skilled and dedicated team of advanced nurse practitioners and emergency care practitioners provides acute visits to patients in their own homes or care homes. The service enables patients to be cared for at home when it is clinically safe to do so and helps GPs manage urgent care needs, enabling them to focus on other priorities in primary care.
The acute visiting service supports:
